INSURANCE
Dr. Myers does not file for insurance payments.
It is the client’s responsibility to submit receipts in order to obtain any reimbursement from the insurance company.
Upon request, the client will be provided with a receipt that includes the codes and information required for reimbursement from companies such as
Blue Cross Blue Shield, or other out-of-network therapy services.
NOTE: Dr. Myers is not a provider for Medicaid or Medicare.
DO YOU KNOW WHAT YOUR INSURANCE COVERS?
It is the client’s responsibility to handle any and all insurance issues directly with the insurance company. Please check your insurance coverage carefully by asking the following questions:
- Do I have mental health benefits?
- What is my deductible and has it been met?
- Do I have a different out of network deductible?
- How many sessions per calendar year does my plan cover?
- How much does my plan cover for an out-of-network provider?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
- Does my plan cover couples and family therapy? Or does it ONLY cover individual therapy?
